(1) to submit to UOB and/or third-party solution providers, including any of their respective agents, dealers and sub-contractors, (the “Partner”) all the information on this form;

(2) to UOB and/or its Partner(s) collecting, using and disclosing the information provided in this form to such persons as UOB and/or its Partner(s) may so decide;

(3) consent to the collection, use and disclosure of my personal data and contact details, as well as those of the representatives named in this form, by UOB Group and/or its Partner(s) for the following purposes:

(a) contacting me/us/the Entity by telephone, email or other communication channels regarding the solutions referred to above (including third-party solutions);

(b) providing me/us/the Entity with such solutions (including third-party solutions);

(c) contacting me/us/the Entity about any UOB or the Partner’s products, services, offers and events that UOB or the Partner thinks may be of interest to my representative(s), the Entity and/or myself; and/or

(d) complying with applicable laws, regulations and the requirements of any relevant authorities; and

(4) that the Partner(s) are solely responsible for their products and services and UOB shall not be construed as endorsing or recommending any product or service provided by the Partner(s) and makes no representation or warranty, whether express or implied, in respect of any such product or service; and that UOB accepts no responsibility or liability whatsoever for the completeness, accuracy of, or for any use, purchase and/or subscription of, any products or services provided by the Partner(s).
